Office Policies

• If you are a new patient, please come to the office 15 minutes earlier than your scheduled appointment to fill out new patient paperwork.
• If your insurance company necessitates a referral for your visit, please obtain one before your visit.
• All co-payments must be paid in cash.
• Any missed appointments without a 24-hour cancellation notice, will be subject to a $25.00 fee.
